Clinic appointment access and fairness
Practise making a fair decision when access is limited. Review whether you identify missing information, weigh both patients' needs, and explain the next step clearly; then record the scenario again with one improvement.

Admission requirements
Applicants need a bachelor's degree before matriculation and at least 90 credits from an accredited institution. Required courses, each with a grade of C or higher, are six semester hours of English and eight semester hours each of general biology or zoology with lab, physics with lab, general or inorganic chemistry with lab and organic chemistry with lab. One year of mathematics plus biochemistry, biostatistics or statistics is strongly recommended. Requirements
Transcripted AP credit can count toward required coursework but cannot replace laboratory work; advanced science in the same area is recommended. The school prefers traditional four-year institutions and traditional classroom science. Applicants educated outside the U.S. should complete one academic year at an accredited U.S. institution and may need a course-by-course NACES evaluation and English-proficiency evidence. Requirements
The MCAT is required. For Fall 2027, accepted dates are August 1, 2024 through September 12, 2026. All scores are reviewed and no application is reviewed without a score. No current minimum GPA or MCAT is published. MCAT
Downstate accepts a committee or adviser letter or packet. Individual-letter applicants may submit no more than two letters and at least one must be from science faculty. Application procedures
New York residency is not required. Preference is given to U.S. citizens and permanent residents who are New York residents, while qualified out-of-state applicants are considered. Downstate says international files are reviewed only after U.S. citizen and permanent-resident files and notes that the class has filled before international review for the last 23 years. FAQ
Important dates
AMCAS opens June 1, 2026. The regular AMCAS deadline is December 1, 2026, and the complete school file is due January 4, 2027 at noon. A complete file includes verified AMCAS, MCAT, secondary, $100 fee or Fee Assistance waiver and letters. Current deadlines Procedures
Downstate begins regular offers in early February and alternate-list decisions in mid-April. The current table lists February 20 through April 30 for Plan to Enroll, April 15 to narrow to three offers, April 30 for the deposit and school selection, July 15 for Commit to Enroll and August 3 as the end of the process. Current deadlines
A stale 2025–26 table remains elsewhere on the procedures page; the cycle-explicit 2026–27 table controls this guide.
How selection works
Downstate uses a holistic, modified rolling process. Current policies and review pages identify academic preparation, prerequisites, MCAT, communication, character, personal skills, health-related experience and motivation, with nonmedical community and service experience desired. Review Policies
The school does not publish numeric screening thresholds, component weights, interview scoring anchors, committee-vote rules or a formula for offers from the alternate list.
Interview details
Confirmed
Selected applicants receive virtual interviews after Labor Day through the first half of spring, generally Monday through Thursday. Decisions for many interviewed applicants begin in February, while later interviews can receive decisions in late spring. Review FAQ
Not published
The current admissions pages do not publish the interview count, duration, structure, interviewer roles, file access, question domains, rubric or numerical weight. The current virtual-interview paragraph retains pandemic-era wording, so invitation instructions should control operational details.
Who this program may suit
Downstate looks for academic readiness, communication, character, health-related exposure, motivation and service. Applicants should connect their preparation to specific responsibilities and to working with patients and communities in an urban public medical-school setting. Review
This guide covers the regular MD only. Do not import Early Decision, MD/PhD, MD/MPH or MD/MHA deadlines.
Practice and next step
Record a 90-second response about serving a community whose needs or context differed from your own. Explain how you learned what mattered, what you did and what you would improve. Review for specificity and humility, adjust one weak section, record a second response and compare the result.

Recommended preparation
Structured Interview Essentials
Build a repeatable spoken-answer loop across five common structured-interview skills: fair decisions, ethical reasoning, using feedback, teamwork under pressure, and explaining your motivation. Record each response once, choose the weakest one, review one observable weakness, and answer it again with one deliberate adjustment.
